Case 58
This young woman had rhinoplasty in Toronto. She disliked the hump on her nose and felt the tip was a bit dependent pointing downwards which gave the nose excessive length. This downward orientation of the tip is often accentuated when you smile. The facial muscles in some people pull down and widen the nasal tip with smiling. This is why many patients dislike their nose in photographs more than in the mirror. She underwent an endonasal rhinoplasty where all the incisions are on the inside of the nose. One can see that the hump on the bridge has been reduced…

Case 67
This young woman had rhinoplasty in Toronto. Before surgery she was concerned about the bump on the bridge of the nose and that the nose was too droopy. She was found to have a prominent nose with a dorsal hump and the tip definitely pointed downwards with a lot of excess in the infratip lobule region ( hanging piece in between the nostrils). The tip was also noted to be a bit bulbous although not out of balance with her prominent eyes. A closed rhinoplasty was performed to reduce the hump, rotate the tip and reduce the hanging infratip lobule.…

Case 66
This young man was unhappy with the bulbous appearance and disproportionately large size of his nose in comparison to the rest of his face. The tip of his nose appeared especially droopy and bulbous from the front. Adding to his problems, his nose was deviated, causing severe breathing problems that added physical discomfort to his self-consciousness about his appearance. To find out more about how cosmetic surgery could correct these problems he planned a consultation with Dr. Rival, rhinoplasty surgeon in Toronto. After discussing a number of options they decided together on an open-approach septorhinoplasty. This procedure is commonly used…

Case 65
This young Toronto woman had a difficult nose. The main issues were with the tip of the nose. It was quite ptotic (droopy) so the goal was to lift the tip and reduce the hanging collumella seen in the frontal view ( the piece in between the nostrils). The concern was that her nostrils were quite retracted and angled upwards so over exposure of the nostrils was a concern when the tip was lifted. To avoid this problem grafts of cartilage were placed along the rims of the nostrils to push them downwards. These are called alar rim grafts. In…

Case 64
This woman underwent rhinoplasty in Toronto. She was concerned about the large hump on the bridge and felt her nose was too wide. She particularly felt the bridge was too wide near the top. She underwent an external approach septorhinoplasty. The hump was reduced and the bridge and tip were narrowed. The narrowing of the tip was somewhat limited by her thick skin. She does however show nice narrowing of her bridge. The nasal bridge is narrowed in rhinoplasty by performing what surgeons call osteotomies. These are fine cuts made in the bone that allow the bones to move inward…

Case 62
This young woman suffered a nasal fracture. This resulted in the formation of a large hump on the nasal bridge near the top. There was also collapse of the bridge near the bottom where the bridge meets the tip ( the supratip area). A reconstructive rhinoplasty was performed to help restore the nasal contour. The dorsal hump was reduced and at the same time a cartilage graft taken from the nasal septum was used to rebuild her bridge. The tip was set back as well a small amount. While most nasal humps are genetic it does seem that some form…

Case 61
This young Toronto woman disliked the appearance of her nose. She felt the tip was too wide and bulbous and prominent on her face. She was bothered by the bump on the bridge. She desired a more sloped bridge on profile. An external approach rhinoplasty was performed to allow significant tip work to be performed. She had relatively thick skin but despite this a significant improvement in tip definition and tip narrowing was accomplished. The bridge was reduced to the desired level to produce a more sloped soft feminine profile. The view from below shows the significant change in tip…

Case 60
This young woman had rhinoplasty in Toronto. She disliked the hump on the nasal bridge and felt her tip pointed downwards. She was particularly concerned about the appearance of her nose in profile view. She underwent an endonasal rhinoplasty (no external incisions). In this procedure the nasal tip cartilages are “delivered” to the outside and modified (see how we do the surgery section). In the after photos one can see that the dorsal hump has been reduced. The tip has been rotated upwards as requested without causing too much nostril show from the front. There is some mild refinement of…

Case 6
This Toronto patients nose came too far out from the face or was overprojected. Conservative rhinoplasty was performed to set the tip back and maintain a strong male bridge. Rhinoplasty surgeons would call this nose a “tension nose.” The nose is pushed forward from the face by the overlong septum and nasal spine ( a small piece of bone at the base of the nose). By reducing these structures the nose fall back closer to the face and is therefore deprojected. One can also appreciate a bulge or ridge in the before picture in the right nostril. This represents the…
